Clinical Analysis of the Cholesteatomatous Otitis Media


Abstract
A clinical analysis was performed for 66 cases of the cholesteatomatous otitis media, observed in Chungnam National University Hospital during the period from 1974 to 1976.
The results were as followings:
1. The incidence of cholesteatoma was 10. 4% of the chronic suppurative otitis media and more common in male.
2. Fifty percent of the cases were observed in age group of 4^20 and the most of the cases (83.3%) in age group of 11^J30.
3. Half of the cases had history of otorrhea for more than .10 years but there were 7. 6% of the cases who had no history of otorrhea.
4. On otologic findings, marginal perforation of the ear drum was most common (posterosuperior perforation in 18. 2% and attic perforation in 28. 8%) and 31. 8% of the cases showed total defect of the ear drum. Purulent otorrhea was observed in 86. 4% of the cases but there were 9. 1 % of the cases without discharge. Granulation or granulomatous polyp on the tympanum or the margin of perforation was observed in 30.3% of the cases.
5. On bacteriologic study, Proteus(28.6%), Staphylococcus .aureus(26.5%), Pseudo¡þmonas(22.4%) and E. coli(14.3%) were observed and cholesteatoma was found in only 31.8% of the cases on the simple mastoid X-ray films.
6. The incidence of complications was 48.5%; subperiosteal abscess in 24.2% (including 2 cases of Bezold¢¥s abscess and one case of squama perforation), facial palsy in 6. 0%, labyrinthitis in 9. 1%, meningitis in 6. 0% and brain abscess in 3. 0% of the cases.
